You will need to create an account if you are a new user- you will require a valid email address and mobile phone number to submit your eAdmission online.
Select ‘Create Account’ on the left side of the screen.

You will get a warning in red text if your email address is already in use, or if your password does not meet requirements.
Select ‘Send me my validation text’ and enter in the validation code sent via SMS to the user’s mobile phone.
Passwords need to be at least 8 characters long and must contain at least one upper case character and 1 numeric character.
St Vincent's Health Australia recommends that you DO NOT use the ‘remember my password’ function. Please enter your user name and password each time.
If you have forgotten your password – simply go to the front introductory page and select the ‘Forgotten Password' link.
You can create a profile for multiple family members within your account. Simply login into the eAdmissions website and select ‘Add New Patient’. Complete all mandatory fields to set up your family members profile details. Once completed select ‘New Admission’. Complete both the Admission Form and Clinical History Form for your family member.
Yes, you will need to complete an eAdmission each time you come into hospital. When you log into your account you will see a list of your previous admissions. Clicking ‘New Admission’ will commence a new eAdmission, which will be prefilled with your previous information. You simply need to confirm the information and make any changes since your last admission.
Please type in the first letter of the doctors surname and wait for the doctor list to load (this is to ensure you have correct spelling). Once the list has loaded, you can continue to type additional letters of the surname to further narrow it down. If the doctor still does not exist sometimes refreshing, the page can help. If the doctors name is still not appearing please contact the hospital on 1300 052 602 further assistance. If the phone number goes to voicemail please leave a message and clearly state your name, contact number and reason for calling.
If your doctor has not provided you with the "MBS Item Numbers" please just skip this question.
Please scroll up the page to ensure you have responded to all questions. Any missing responses will have red text next to it prompting for an answer. Complete and select Save and Continue.
If you cannot locate your GP details, click on the box ‘ I cannot find my GP’. This will allow you to manually type in the GP’s details including their address. It is a compulsory field to enter your GP’s address.
You can delete the GP listed by clicking on the ‘x’ icon. This will remove the existing GP. You can then add your new GP.
Yes, you will receive an email confirmation after completing the eAdmission. Please check your ‘junk mail’ if it is not in your inbox.
No, it is not possible to update your forms once they have been submitted. Please contact the hospital on 1300 052 602 where staff can make changes on your behalf.
Yes, before you submit your eAdmission you have the option to ‘View Form’ for both the Admission and Clinical History Form. Click on the icons (shown below)
This will open the information in PDF view – which you can then save if you wish to retain a copy.
If you wish to view this information, after you have submitted your eAdmission, from the homepage, select the 'View Form' button against the admission you wish to access, and 'Print' to download a PDF copy.
